About Us

Hudson Headwaters Health Network is a nonprofit network of community-based primary care health centers serving patients of all ages from Saratoga County to the Canadian border. The Network’s Ticonderoga Health Center is located within the state-of-the-art UVM Elizabethtown Community Hospital – Ticonderoga Campus and offers primary care to people of all ages. Our team includes family medicine practitioners, pediatricians and behavioral health professionals who are here for you through every age and every stage. In addition to our health center, the medical campus features an emergency room, laboratory and radiology services, physical therapy and sports rehabilitation, and a long-term care facility.
